SIH WorkCare is dedicated to providing patients with a high level of quality services. We care about the safety, health and well-being of our patients and we want to ensure our patients get the appropriate level of care.

What to Expect During Your Visit:

Your employer may have asked you to participate in a Pre-employment physical. The physical will be in compliance with ADA and EEOC standards. The physical may include some of the following tests:

• Medical physical Exam
• Drug Screening
• Hearing test
• Respiratory test
• Visual test
• Lab/Blood test
• Physical job demand test:
  • Lifting
  • Push/Pull Testing
  • Bend/Squat/Kneel

Please be prepared to be at our clinic for 1- 2 hours based on your specific company’s testing requirement.

Instructions for arriving for your Physical Exam:

• Please bring a photo ID.
• Wear comfortable clothing similar to work attire.
• Wear footwear similar to work footwear or tennis shoes. No sandals, flip flops or dress shoes.
• Avoid smoking and consuming caffeine 1 hour prior to appointment time.
• If you are ill the day of your test, please contact our office prior to your testing time.
• If you are receiving or have recently received medical attention for an injury, or had surgery, please contact us for further instructions at 618.993.3817.

DOT Physical:

• Bring a copy of your Current CDL that matches your current address
• If you are diabetic, please bring a copy of your last A1C
• If you wear a CPAP machine for sleeping, please bring your last 90 days of readings
